This representative model reflects the multi-site scaling challenge directly: a regional group with six locations and 22 providers wanted a single, consistent CCM and RPM program rather than the informal, inconsistent outreach a few individual physicians had tried on their own.

Rather than launch at all six sites simultaneously, the rollout began with two locations selected for their engaged physician leadership and representative patient mix. Elixir used this phase to finalize enrollment scripts, escalation thresholds, and documentation workflows against real operating conditions.

With the two-site model validated — consistent enrollment rates, clean documentation, and no clinical escalation gaps — the same workflow, QA process, and reporting structure were extended to the remaining four locations over the following months, with each site's performance tracked individually against the same benchmarks.

The result modeled here is a network where the sixth location performs as consistently as the first — the specific outcome multi-site groups most often cite as their biggest risk with any new clinical program.
